If you are interested in participating in Los Altos HS athletics, please review the information in this section. All LAHS athletes must complete the Clearance process for each sport, every year.
Before you can participate in LAHS team tryouts or practices you must complete the online clearance process and submit the confirmation letter and Physical/Insurance docs. These forms must be uploaded and cannot be emailed.
Academic Eligibility:
To be eligible to participate in extra and co-curricular activities, students must:

earn at least a 2.0 grade point average.

have completed 20 units of credit in the previous grading period (for returning students)

be currently enrolled in a minimum of five classes
-
be passing four of the five classes

Double Sports:
An athlete may participate in only (1) LAHS sport per season. There are no exceptions.

REGISTRATION FOR 2024-2025 ATHLETICS IS OPEN
Before you can participate in LAHS team tryouts or practice you must have completed the online clearance process. You will receive a confirmation letter to confirm your registration.
Step 1 - Go to homecampus.com/login
The online form/process includes Emergency Information, Medical History, Sports Physical Forms, and Eligibility Requirements (athletes must have a minimum 2.0 GPA in the current grading period and no more than 1 "F" to be eligible), Student/Parent Handbook, as well as the Co-Curricular Drug & Alcohol Policy and Concussion Information.
Step 2 - Sign in to your account or register to set up an account
Step 3 - Select the correct year and school
After you have logged in:
- SELECT year - “2024-2025”
- SELECT school - Los Altos (CIF-CCS)
There are 2 Los Altos high schools on the list, make sure to select the one with (CIF-CCS) after the name. - SELECT sport for clearance - Choose the sport you are trying out first.
Step 4 - Complete the online clearance
Now you will be guided through the clearance form/process in a series of steps.
At the end of each step/page, you hit the SAVE button.
If you have not completed all the required fields, a red box will appear where you still need to input information. Complete that information, then click SAVE to continue to the next screen. The form goes through the following sections:
- Personal, insurance, and physician information
- In this step, download the MVLA Physical Form.
- The Physical Exam Sheet MUST be signed, stamped, and dated by a physician with the date of the exam.
- In this step, download the MVLA Physical Form.
Step 5 - Upload your completed Physical, Insurance Form, and confirmation letter.
YOU WILL BE DENIED IF PHYSICAL and INSURANCE are not uploaded. Please upload in PNG, JPEG format.

STEP 1: Athlete and parent review, agree, initial, and sign off on the form.

STEP 2: Print & attach a copy of your class schedule

STEP 3: Submit to the Athletic Director for signature upon verification of making the team
|
STEP 4: Get PE teachers' signature
|
STEP 5: Submit to Attendance for approval and notation for SIS attendance
|
STEP 6: Wait until the designation of OTHER in the SIS system or verbal clearance by the PE teacher
|
At the conclusion of the athlete's season, they MUST return to PE immediately following the last day of competition.
This process must be done for EACH season of sport.
